Mental Health Medical Records Requests

If you need a copy of your medical records from Johnson County Mental Health Center, please print and complete the Authorization to Disclose Protected Healthcare Information (English) or Autorización para Divulgar Información de Atención de Salud Protegida (Español) and return it to JCMHC by mail, email or fax.
